As Western New York’s true locally based health plan, Independent Health recognizes our members are more than our members. They’re our neighbors, our kids’ coaches, our pickleball partners, book club members and our friends. Because of these connections, we want to make sure our members have the information and resources they need to get and stay healthy.
Here are just some of the many resources, programs and community initiatives we have put in place to help our members get care, guidance and support.
Personalized preventive health reminders
Independent Health delivers personalized messages to members to encourage them to get their preventive screenings and services. These reminders are tailored to the individual based on their age, gender, health status, claims and other factors specific to a member, and are aligned to clinical standards and recommendations. Members with a portal account can view and track their personalized member action plan of the recommended preventive services they need to have. This personalized list of action items helps a member partake in healthier behavior and better decision making.
Brook+
Independent Health offers Brook+ as a way to help our members who may have prediabetes take charge of their health, achieve a healthy weight, and reduce their risk of developing type 2 diabetes. This year-long, CDC-recognized Diabetes Prevention Program is completely digital, which means individuals access it using a smartphone, tablet or computer. No classrooms or clinic visits are necessary. Eligible participants are provided with the tools and health coaching they need to help them make realistic and sustainable lifestyle changes. Plus, it’s available at a $0 member cost share.
Care for You program
To address the specific and extensive care required by individuals with multiple chronic conditions, Care For You involves a dedicated care team of physicians, physician assistants, nurse practitioners, registered nurses, social workers, dietitians and community health workers who work with patients to develop individualized, proactive care plans in concert with their primary care physician (PCP).
The program supports alignment among physicians and care personnel, providing a single point of support through centralized services. Care For You patients also undergo an assessment of social determinants of health so the care team can link them with applicable community resources and educate and promote medication adherence.
Unique benefits
Our benefit plans offer unique benefits to help members make it easy for them to make healthy choices and decisions. From making it more affordable to purchase fresh produce, join a gym or visit their primary care physician, our programs are designed to help members achieve their personal health and fitness goals. Depending on one’s health plan, we offer features like a nutrition benefit or a $250 debit card to use on fitness club memberships, massage therapy, vitamins and other services.
Community events
We want to help the community’s health as well, which is why Independent Health and the Independent Health Foundation work with a variety of local partners around Western New York to offer easy access to healthy activities. From bikeshares to fitness classes and wellness walks, there is no shortage of ways to combine fitness and fun.
Our partners include the YMCA Buffalo Niagara to offer Fitness in the Parks; Buffalo RiverWorks, Reddy Bikeshare, and more. These programs and initiatives are just a sampling of all the ways we support our members and the entire community.
